David U
David U received his B.Sc.Phm and M.Sc.Phm. degrees from University of Toronto. He has extensive
experience in hospital pharmacy management as well as in health care technology. David's keen interest
and passion in medication safety provided a solid foundation for him and Michael Cohen of ISMP (US) to
found the Institute for Safe Medication Practices Canada (ISMP Canada) in early 2000. Like its sister
organization in the States, ISMP Canada intends to serve as a national resource to promote safe
medication practice throughout the healthcare community in Canada. David U has written many articles
and columns about safe medication practices and has also given many presentations on medication errors
and their prevention strategies both abroad and within Canada.
Presently David is a member of the Patient Safety Advisory Committee of the Canadian Council on
Health Services Accreditation, member of the Ontario Ministry of Health Patient Safety Task Force,
member of the Ontario Hospital Association Patient Safety Advisory Committee, member of the Ontario
Chief Coroner Patient Safety Review Committee, and member of the Safer Healthcare Now Steering
Committee.
David is the President and CEO of the Institute for Safe Medication Practices Canada. Since January
2001, David is also a Board of Trustee of the Institute for Safe Medication Practices (US).
