What Should be Reported?


A medication error is a mistake with medication, or a problem that could cause a mistake with medication. You can report any medication error involving the use of prescription and non-prescription medications, natural health products, imported products and devices or equipment used to administer medications.

Any medication error that happened to you or to someone you know, can be reported. If you are reporting on behalf of someone you know, you should ask their permission to report the error and be sure that you know all the facts about the error.

Adverse drug reactions (also known as "side effects") are not medication errors. Adverse drug reactions should be reported to Health Canada's Canada Vigilance Program.
